Considerations for disability characteristics For information intake, we ask the parents (guardians) about child’s behaviors and communication aspects that make them concerned including those that make them a bit anxious.
Expected effects and results The collected information is to be used for correct understanding of the actual states as well as for subsequent parent interviews. In our school, the records are also reflected in the “Support Book (developmental and behavior record book for children with developmental disability provided by municipal governments)” of the individual.
How to use According to circumstances, ask the parent (guardian) to fill in the sheet in order to correctly understand the actual states.
